Driver Qualification Folders – even if you own the truck you need this on yourself

  • Application
  • Previous Employment Requests
  • FMCSA Drug & Alcohol Clearinghouse results & Authorization https://clearinghouse.fmcsa.dot.gov/
  • Abstract – driving record
  • Pre-Employment Drug Test and proof of random pool enrollment
  • Yearly MVR and Reviews (yes even if it is on yourself)
  • Current Medical Cert
  • Copy of Driver’s License
  • Write-ups and accident reports – if a driver receives a ‘Driver Behavior BASIC’ violation – it is in your best interest to do a write-up.

Equipment Folders

  • Registration
  • Yearly inspection proof
  • Insurance
  • Preventative Maintenance Schedule
  • Proof of repairs and maintenance
  • Accident Reports
  • Road side inspection reports and proof of repair
  • DVIR’s write-ups for the last 90 days

Accidents

  • Know the difference between and DOT Reportable and NON-Reportable accident
  • Complete your ‘Accident Register’ every year and retain for 3 years
  • Include accident report, road side report, loss run, and all other required information
  • If a drug/alcohol test was required, keep a copy of the request

Controlled Substances

  • Have a current driver list for the random pool
  • Retain past 3 years random pool lists
  • keep all verified results for 3 years
  • Pre-employment results must be kept in the driver qualification file
  • Proof of Clearinghouse enrollment
